Invoice with your company name, remittance address and itemized accessorial charges. Signed Bill of Lading with Proof of Delivery. If a load has multiple orders or pages on BOL, all pages must be submitted along with inspection forms. Any Valid unloading/lumper, pallet, gate, etc receipts must be submitted with original invoices for reimbursement, even if a Come check has been provided in advance.
